An opportunity has arisen for Social Workers experienced in undertaking NHS Continuing Healthcare DST’s.
The Successful candidate must have recent experience undertaking CHC related work.
They will be responsible for completing Care Act Assessments prior to the DST meetings, Mental Capacity Assessments and related commissioning work if individuals are no longer eligible for CHC.
A thorough working knowledge of the NHS Continuing Framework is required, and a practical understanding of the Adults Liquid Logic (LAS) IT system is essential.
