An Adult’s Social Worker/Advanced Practitioner is required for the Community Discharge to Assess Team in Warrington.

In this role, you will be responsible for conducting comprehensive Care Act assessments for individuals who have been discharged from hospital into a temporary 24-hour care placement. Your primary objective will be to assess the individual’s care needs, identifying appropriate services, and coordinating the care and support. This is a fast paced role, where timely and quality assessments are needed. As a vital member of our social work team, you will play a crucial role in supporting individuals to achieve their outcomes.

Candidates must have D2A experience to be considered for the role.

Responsibilities: 

  • Conduct Care Act assessments of individuals who have been discharged from the hospital and require community support.
  • Assess and evaluate the individual’s physical, mental, and emotional needs, as well as their functional abilities, to determine the appropriate level of care and support required.
  • Collaborate with individuals, their families, and other healthcare professionals to gather relevant information and develop comprehensive care plans.
  • Provide advice, guidance, and emotional support to individuals and their families, helping them to understand their options and make informed decisions.
  • Liaise with relevant community resources, such as healthcare providers and voluntary organisations, to ensure the seamless provision of services.
  • Advocate for individuals’ rights and entitlements, ensuring that they receive appropriate and timely support services.
  • Maintain accurate and up-to-date records of assessments, interventions, and outcomes in accordance with organizational and legal requirements.
  • Stay current with changes in legislation, policies, and best practices related to social work, care assessments, and community support services.

  • DIPSW or equivalent qualification in Social Work
  • Social Work England registration
  • Post qualification experience
  • Enhanced and in date DBS certificate.
  • Excellent communication skills and be able to make good decisions based on information available.
  • One year minimum post qualified experience in Social Work
  • Can work alone and within/contribute to a team/multi-agency set ups.
  • Possess knowledge of service needs.
  • Competent IT skills and be able to navigate electronic file/recording systems.
  • Can write clear, concise, and robust assessment/reports/files.
  • Have a flexible approach to work and working hours when the need arises
